Scheduling Appointment: Appointments can be scheduled via phone or email. Please book routine draws, nursing home draws & courier services at least 24 hours in advance to ensure availability. STAT draws can be requested by calling us at (844) 490-POKE (7653).
Appointment Confirmation:
All appointments will be confirmed via email or phone 24 hours in advance unless booking an appointment for the same day. Please ensure your contact details are up to date.
Cancellations and Rescheduling:
To better accommodate our patients we ask that any cancellations or rescheduling requests be made at least 24 hours before the scheduled appointment.
Refunds:
We understand that plans can change, and we aim to provide a fair and straightforward refund process. Refunds will be processed within 48 hours of cancelling your appointment. You must cancel your appointment at least 24 hours in advance to receive the refund. All refunds are subject to a $10 administration fee. This fee will be deducted from the total amount to be refunded. Refunds are not available for no-shows or cancellations made within 24 hours of the scheduled booking time.
Payment:
Payments can be made online after receiving our invoice. Invoices are sent by email after appointment is scheduled. Please make sure invoice is paid before your appointment. If you wish to pay in cash, please let us know at the time of scheduling your appointment. Payment is due on the arrival of the phlebotomist and before any services are rendered.
Insurance:
Our mobile phlebotomy services are not covered by insurance. However, we require that you please provide your insurance information for billing purposes for any testing completed by the lab. If you receive a bill from the lab, please contact them directly.
Privacy and Confidentiality:
All personal and medical information provided by clients will be kept confidential and used solely for the purpose of providing the phlebotomy service. Our services comply with all relevant privacy laws and regulations.
Liability: We are committed to providing professional and safe services. However, we are not liable for any adverse reactions or complications that may arise from the blood draw procedure.
Service Scope: Our mobile phlebotomy service involves professional blood draw services conducted at the clients location. Services are provided by certified and trained phlebotomists.
Client Responsibilities: Clients must provide a safe and appropriate environment for the blood draw. Clients must inform us of any medical conditions or special requirements at the time of booking.
Changes to Terms and Conditions: We reserve the right to update or modify these terms and conditions at any time.
By booking our mobile phlebotomy services, clients agree to these terms and conditions.
We ask that before booking your appointment you please email or upload your doctor requisition form, insurance card and ID so that we may prepare for your appointment and provide the lab with all the necessary documents needed to process your specimens.
